Hospitalist Program Services

A Hospitalist takes over the complete direct response care and monitoring of a patient after they are admitted to the Hospital. They allow primary care physicians to focus on outpatient practices, while knowing that their patients are receiving round-the-clock care. Beyond caring for inpatients in the Hospital, Hospitalists’ responsibilities also include care for patients of doctors not currently affiliated with the Hospital. These committed physicians take care of the patient in every aspect until they are discharged. Since they have direct continuous monitoring contact, they can record test results and data updates, and act immediately in response to changes. The Hospitalist also keeps the patient’s primary care doctor informed of any changes. They are on-site at the Hospital every day, so they can answer family questions quickly and efficiently with minimum interruption to patient care. Quality patient care is continuous and consistent.

The greatest benefit of the Hospitalist program may be that in many cases, the patient’s length of stay in the Hospital is reduced between 15 to 50% as a result of the careful and continuous care the service provides. This enables patients to return to their home to continue their healing process in familiar and more comfortable surroundings.

Hospitalists are the primary information source for patients and family member regarding care issues. They recognize potential problems and respond rapidly to changes in the patient's condition They improve communication between care professionals during shift changes. Many times they are called upon to facilitate connections with post care organizations such as home health care, skilled nursing or rehabilitation groups.
