Returning Junior Volunteer Application Checklist

Step 1: (to be completed by February 15)

Enrollment is now closed for the 2013 Junior Volunteer Program.

Step 2: (to be completed by March 29)

 Phone interview with Volunteer Coordinator.

Step 3: (to be completed May 11)

 Attend a volunteer orientation (students under 18 must attend with parent/guardian).

Step 4: (to be completed by May 11)

Complete annual Tuberculin Skin Test and update immunization records

Step 5: (to be completed by June 7)

If you need to re-obtain an ID badge, please do so.
If you need a new uniform, please purchase one at Volunteer Services Office

Optional Step 6: (to be completed by May 31)*

FBI Fingerprinting and Child Abuse Clearance forms mailed to the appropriate government agencies. When you receive your clearance certificates back from the agencies, the Volunteer Services office will need copies of them.

*These clearances are ONLY required for those who are volunteering in the Child Development Center, Children's Health Center, Emergency Department, Women's Health Center, Detox Center, or Center for Mental Health. Please note that the Child Abuse Clearance may take up to 3 weeks to process.
