First-Time Junior Volunteer Application Checklist

Step 1: (to be completed by February 15)
	Enrollment is now closed for the 2013 Junior Volunteer Program.
	Print out and send in two letters of recommendation to the Volunteer Services office.
Step 2: (to be completed by March 29)
	Interview with the Volunteer Coordinator .You will be contacted for an interview when Volunteer Services has received your completed application and two letters of recommendation.
Step 3: (to be completed by May 11)  Attend volunteer orientation (students under 18 must attend with a parent/guardian). Step 4: (to be completed before orientation)
	Complete mandatory two-step Tuberculin Skin Test and submit immunization records to Employee Health Services.
Step 5: (to be completed by June 7)
	Obtain ID badge (requires signature from Volunteer Services and Human Resources)
	Purchase uniform at Volunteer Services Office
Male:  Female:          Short Sleeve Polo  $14  Short Sleeve Polo  $14       Optional Step 6: (to be completed by May 31)* FBI Fingerprinting and Child Abuse Clearance forms mailed to the appropriate government agencies. When you receive your clearance certificates back from the agencies, the Volunteer Services office will need copies of them. *These clearances are ONLY required for those who are volunteering in the Child Development Center, Children's Health Center, Emergency Department, Women's Health Center, Detox Center, or Center for Mental Health. Please note that the Child Abuse Clearance may take up to 3 weeks to process.
