TCC - What to Expect

A team that includes an internal medicine physician, nurses, case manager, and, as needed, occupational therapist, physical therapist, speech therapist, social worker, dietitian, recreational therapist, and psychologist will develop an individualized program for treatment. The team helps set patient goals and encourages patients to work toward those goals in both individual and group therapy.

Customized therapy sessions for Transitional Care Center patients are less intense than on other units within the Hospital. Our physicians, or hospitalists, see patients as needed (possibly  2-3 times per week) in the TCC.
