The Transitional Care Center (TCC)
The Transitional Care Center is a fully licensed and regulated, 50-bed skilled nursing care unit located on the First Floor of Reading Health Rehabilitation Hospital. The TCC provides skilled medical and rehabilitation services that act as a transition – or bridge – between acute care and home. The center is staffed by our Internal Medicine physicians, with consultations to RPS Physiatry as needed.
The Transitional Care Center serves patients recovering from the following conditions:
- Cancer
- Cardiopulmonary conditions
- Generalized weakness
- Multiple trauma
- Neurological or neuromuscular disorders
- Orthopaedic conditions including joint repair or replacement
- Stroke or aneurysm
- Surgery
