Reading Health Family Medicine and Internal Medicine Practices Have Received National Committee of Quality Assurance Recognition

Reading Health System is proud to announce that all of its longstanding family medicine and internal medicine practices have received the National Committee of Quality Assurance's Patient Centered Medical Home™ Recognition at Level III, the highest level of recognition available.

Patient Centered Medical Home is a care-delivery model that emphasizes care coordination and communication in primary-care practices. Research has shown that medical homes can lead to higher quality and lower costs, and can improve patients' experience of care.

With guidance from a Reading Health Patient Centered Medical Home leadership team, each of the 22 practices developed a sustainable model of patient-focused, team-based care. They incorporated daily team huddles, pre-visit planning to fill care gaps in preventive care, and active management of very high-risk patients with the assistance of nurse care navigators.

The National Committee of Quality Assurance is a private, non-profit organization dedicated to improving healthcare quality. This recognition acknowledges that these Patient Centered Medical Homes offer exceptional patient care and contribute to Reading Health's clinical excellence.
